Position: Life Coach III / Day Program DSP Salary: $18.82 per hour Schedule: Monday thru Friday Locations /Job Type: Hamden Full-time , New York Responsibilities: Support people to live personally fulfilling lives and achieve their person centered valued outcomes through a positive and proactive approach. Other duties: Foster independence through role modeling, teaching, and supporting people to be valued and contributing members of their community Talents are identified and connections are made in their community, based on interests and desires Reports, log notes and documentation are completed in an accurate and timely manner Managing and setting up reminders to keep track of deadlines for reports/paperwork Excellent Communication skills Strong attention to detail Innovative problem solving skills Entering/uploading data in Medisked Provide support in other areas of the agency Passion for helping others, and the ability to build relationships. Qualifications: High School Diploma or equivalent, plus one year experience preferred Ability to lift 40 lbs.
