(restrictions may apply) *The Assessment, Registered Nurse (ARN) is
responsible for assessing prospective enrollees and active health
plan members to determine potential care needs and plan
eligibility. As a hybrid role, the Assessment Nurse starts and ends
their day at home. The nurse completes the assessments in the
health plan member's home or virtually as scheduled by the plan's
Scheduling team.Performance Responsibilities and Standards:Conducts
virtual or in-person visits with potential enrollees and active
members of the health plan as assigned.Provides a comprehensive
overview of program benefits to applicants, their family members,
and caregivers.Educates consumers and members on their rights,
responsibilities, and the services offered by Nascentia Health
Options.Interviews members, family members, or significant others
identified through case findings to assess their service
needs.Reviews the member handbook, provider directory, and covered
services during the visit.Evaluates and reviews previous assessment
information.Performs all assessment activities in compliance with
regulatory agency requirements, completing necessary documentation
and developing initial or updated care plans.Assists in identifying
unmet needs and creating person-centered service plans to address
them.Communicates effectively with potential members, their
families, care management staff, and providers.Collaborates with
the healthcare team to address potential enrollees' needs.Gathers
information on eligibility, benefit coverage, and benefit
coordination.Ensures accurate completion of all required
documentation in the health plan's electronic medical record (EMR)
and quality review programs.Participates in departmental meetings,
as well as scheduled staff meetings and applicable training
sessions.Adheres to all state and federal privacy regulations,
including HIPAA, and Nascentia Health Options policies regarding
confidentiality, privacy, and security.Maintains accurate
scheduling information.Acts as a community liaison for the health
plan.Other duties as assigned.Requirements:Nursing: Possession of a
current Registered Professional Nurse license in New York State is
required. A Bachelor of Science in Nursing (BSN) is preferred,
while an associate degree is mandatory. Previous nursing experience
of one year is preferred.Previous experience in long-term care
and/or managed care settings is a plus.Knowledge of homecare,
Medicare, and Medicaid is beneficial.Bilingual skills are a
plus.Demonstrated ability to work independently and effectively
manage workload priorities.Proficient in computer usage with a
working knowledge of basic computer skills and Microsoft Office
applications.Excellent interpersonal, virtual, and telephonic
communication abilities.Valid New York State driver's license and
ability to drive in all types of weather.Aptitude to comprehend and
utilize the organization's software systems to execute job
responsibilities.Physical Requirements:The role is to work from
home with the requirement to visit health plan members in their
homes. Attendance of in-office work and meetings may be
required.Standard postures include Standing, sitting, bending,
walking, and driving (frequently entering and exiting vehicles
throughout the day).Frequently uses a laptop computer to enter
orders and obtain product information. Assessments are completed
using a laptop to enter the information into the UAS-NY and EMR.
Laptops are also utilized to facilitate the virtual
assessmentsSensory requirements include speech, vision, smell, and
hearing.Lifting requirements of up to 20 pounds frequently and up
to 50 pounds occasionallyBloodborne Pathogens Exposure Category I -
Employee has tasks that routinely involve exposure or potential
exposure to blood, body fluids or tissues.Compensation &
