Responsibilities: Provide high-quality medical care to residents in
a SNF setting, focusing on reducing unnecessary hospitalizations

treatments and medications Collaborate with facility staff,
including physicians, nurses, and other healthcare professionals to
ensure seamless continuity of care Communicate effectively with
families, residents, and healthcare providers to ensure open lines
of communication Stay up-to-date with industry best practices,
regulations, and survey processes to ensure compliance and
continuous improvement Requirements: Salary: $105,000 - $152,000
