Employment Type: Full time Shift: Day Shift Description: Registered Nurse Clinical Lead-Saratoga County The Community Hospice has a wonderful opportunity for a FT RN Team Leader to work as part of our Hospice Care Team caring for patients and families residing in the Saratoga, Montgomery, and Washington County areas. What you will do: - The clinical leader engages in activities that support the planning, coordination and implementation of patient care under the direction of the patient care supervisor. - The RN also carries a small caseload which includes the implementation of an individualized plan of care including but not limited to managing discomfort and providing symptom relief. - This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers) What you will need: - Valid NYS RN license - Associates degree in nursing, BSN preferred - 2 years of experience as a RN required - preferably Hospice homecare, or in an acute care environment - Previous home care / hospice experience preferred but NOT required. - Strong engagement to promote a strong team culture - Valid driver license and reliable vehicle - Understanding of Hospice principles preferred Training provided in end of life care Community Hospice provides a thorough orientation, preceptors and a supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
