WeCare Centers at Foltsbrook Care Center is actively seeking a motivated RNAC for our community in Herkimer, NY . The RNAC is responsible for coordinating the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), care planning, and quality assurance activities. The ideal candidate will have strong clinical assessment skills, knowledge of Medicare and Medicaid regulations, and a commitment to providing high-quality care to our residents.
