Job Description:
TeamHealth has an excellent opportunity for a psychiatric nurse
practitioner (PMHNP) to join our post-acute care team in the Uitca,
New York, area. In this role you will work closely with a social
worker for visits. Expectations are to see all residents on
psychotropics medications as well as behavioral residents and to
participate in GDR meetings of psychotropic medications. Gehrimed
and PCC integrated in all facilities. This role covers 5 facilities
where you will visit one facility every day. If there is a schedule
change you will communicate with the CPM and facility. There are no
on-call hours, and some emergent cases will be called in to
clinician.- Work closely with social worker for visits - 5 days per
week- one facility every day - No on call hours- some emergent
