Helio Health and Central New York Services announce an affiliation and planned merger.Central New York Services and Helio Health have concluded that both organizations will be stronger together in pursuing the shared goal of building a comprehensive and integrated system of care for people with substance use and mental health disorders.GENERAL STATEMENT:The full-time Case Manager will be scheduled 40 hours per week. The Case Manager provides direct support, crisis intervention, advocacy, and support linkages for individuals that meet eligibility criteria.DUTIES AND RESPONSIBILITIES:1. Intake and assessment of eligible participants.2. Individualized Service Plan Development.3. Encourage client participation in clinical, psycho-social, educational, vocational training and other services as indicated in the individualized service plan.4. Supporting and encouraging participants as they adjust to changes in their lives and progress through recovery.5. Linking clients with appropriate support services by communicating with both participants and service providers.6. Responding to and reporting crisis situations.7. Adhering to applicable guidelines and procedures established by HUD, NYS OASAS and OMH.8. Maintains staff/participant confidentiality with regard to written and verbal information.9. Ensures all documentation and entry into the electronic medical record is accurate and timely.Physical Demands: The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision and ability to adjust focus. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl.
